Hi Rory - I don't have the original. A copy was forwarded to me as an e-mail attachment many years ago and I can't even remember who sent it to me. If there's a way to enlarge it I'll happily send you a copy but you'll need to let me know how to do it because my technical skills in that department are pretty well non-existent!
