This is an example of a medal roll for the QSA. Depending on the period for which someone served and in which units he/she served, there could be between 1 and several pages like this one that list entitlement to the QSA. Someone's entitlement to clasps on the QSA come from the QSA rolls and the Supplementary rolls.

All the QSA roll pages follow an almost identical format. (The following images are reproduced under licence with the National Archives)



The page is composed of several sections:

Title

Lists the regiment and battalion to which the roll relates:



|Recipient's details



Clasps

Which of the clasps the recipient has earned. A forward slash can be used to denote entitlement, sometimes the word 'Yes'. Occasionally the clasps name can be over-written with the date clasps.



Notes

Any remarks relating to the recipient. This column can contain details of service in other units which is very useful but can also say of the recipient was killed or died.



Creation place and date

Where the roll was written.



Signature

The signature of the person responsible for the roll, usually the unit commanding officer.



National Archives reference

Not part of the original roll but appended by the National Archives. This reference plus the page number (near the signature area), uniquely identifies a QSA page. In the case of this example, WO100/196p8.
